Please ensure that your submission is completed within 3 weeks of you qualifying or you may risk losing your spot to a reserve.
Please only complete the form once per qualification and do not re-enter any requests for clothing or camping already made. Any changes to your submission will be discussed by email for official confirmation.
You will receive an automated email to the email that you provide in the form from Google Forms with a copy of your responses. Please ensure that the email you provide is correct and that the email on your A4A account is up to date as if these are both wrong we may have no way of contacting you.
